PAYROLL SETUP FORMS AND LINKS

1. To setup your company for payroll processing services
   we will need:
   • Copy of Payroll Services Contract (pdf , doc) , signed
   • Signed IRS Form 2848 (State & Local POA's if required)
   • Copy of a company check on the account from which we will draft payroll. (May be voided)
   • New Client Info sheet
   • Employee Information
   • Copies of Current Yr. 941’s, Copies of State and local returns (if applicable)
   • Copy of IRS document with Company name and Company EIN, for verification
